Use the Primefold XI – Checkout Survey app to display a survey after checkout on your Shopify store's Thank you page. This is the only supported placement.

Overview

With this integration, you can:

  • Install the free Primefold XI – Checkout Survey app from the Shopify App Store
  • Connect one or more Shopify stores to your Primefold XI workspace with an access token
  • Choose which published survey each store shows after checkout
  • Customize button labels, texts, and the answer layout in Shopify's checkout editor. English defaults are pre-filled.

Important: Surveys can be embedded only on the Thank you page after checkout, not on the Order status page, product pages, cart, or other storefront pages.

Before you start

You will need:

  1. A Primefold XI workspace with a published survey. Owners, Admins, and Members can manage the integration. Viewers see it read-only.
  2. Admin access to your Shopify store. Thank you page app blocks are available on all Shopify plans.

Step 1 : Connect your store in Primefold XI

  1. In Primefold XI, open Integrations in the left sidebar under Gather data.
  2. On the Shopify card, click Add store, enter a name for your store, and click Create store. An access token is generated automatically.
  3. In the Survey column, select the published survey you want to show after checkout, then click Save changes.
  4. In the Access token column, click the copy icon. The full token is copied to your clipboard.

Step 2 : Install the app in Shopify

  1. Open the Primefold XI – Checkout Survey listing in the Shopify App Store: apps.shopify.com/primefold-ai-app
  2. Click Install and confirm. The app is free and requests no access to your store data.
  3. The app opens in your Shopify admin with a setup page. Click Open checkout editor. You can also go to Settings → Checkout → Customize and switch the page selector to Thank you.

Step 3 : Add the survey block to the Thank you page

  1. In the checkout editor, choose Add app block and select Primefold XI Survey.
  2. Place the block in the header, main content area, or footer of the Thank you page.

Step 4 : Paste the access token

  1. With the block selected, paste the copied token into the Access Token field in the block settings.
  2. Click Save in the checkout editor.

Step 5 : Customize texts and layout (recommended)

In the block settings, you can customize:

  • Button Text: Next
  • Button Text: Submit
  • Checkbox Instruction Text
  • Comment Placeholder Text
  • Desktop columns
  • Mobile columns

Default language: English (pre-configured).
If your store is not English-first, update the texts before going live.

Questions, branding, and the thank-you message come from the survey itself in Primefold XI.

Verify it works (test order)

To confirm everything is set up correctly:

  1. Place a test order in your Shopify store.
  2. After checkout, on the Thank you page, confirm the survey appears.
  3. Submit a test response and verify it shows up in Primefold XI.

If the block shows "Survey temporarily unavailable", check that the store in Integrations has a published survey assigned and that the token in Shopify matches.

Managing the integration

  • Change the survey: Pick another survey in Integrations and click Save changes. It takes effect immediately, and there's nothing to change in Shopify.
  • Several stores: Add one store per Shopify store. Each has its own token and survey.
  • Regenerate access token: The old token stops working immediately. Paste the new one into the block in Shopify.
  • Delete store or uninstall: Deleting the store in Primefold XI stops the survey. Uninstalling the app in Shopify removes the block. The app stores no merchant or customer data.
